CARE MANAGEMENT SERVICES:
If your insurance company has referred you to TLC Consultant Services, it will be billed
for our services.  If you contact TLC Consultant Services or are referred by your physician
or others involved in your care, your insurance carrier will be contacted.  You will be
billed directly if coverage is not available.

Care Plan Assessment report $600
($800 for couple)

Retainer Fees:
$500.00 (Low Level of Services)
$1,500.00 (High Level of Services)

Hourly Rate $85

TLCCS will save you money in the long run by assessing your precise needs and
helping you choose the specific services that will best serve you now.  Most
people require geriatric care management only intermittently once support
services are in place.

  • Identifying eligibility for assistance and community services

    After discussing and evaluating the current situation and listening to your goals for the future, we will create
    a personalized, plan of care that will meet the needs and/or desires of both the client and the family. If
    ongoing assistance is necessary, we will help you implement it. By offering a full range of support needed to
    remain living comfortably and safely in the home or another location.

ONGOING CARE MANAGEMENT

    When disabled adults, older adults and families need support on an on-going basis, the care manager is an
    advocate who provides oversight, coordination, and support for the older adult(s) and family to:

  • Locate and manage hired home care / caregivers
  • Liaison with medical professionals
  • Enhance confidence and independence
  • Locate needed medical services
  • Coordinate and attend physician visits and interpret results to family
  • Liaison to families at a distance
  • Visit the disabled adult or elder and monitoring the care of your loved one
  • Provide consumer education, negotiation, and advocacy
  • Provide counseling and psychosocial support
  • Assist with home repairs and maintenance
  • Assist with relocation, when necessary
  • Educate elders and their families so informed decisions can be made
  • Offer on-call assistance to the disabled adult, elder and family whenever crises occur
  • Weekly case assessment and management conferences

BILL PAYING AND INSURANCE FILING

  • TLCCS services can assist clients with budgeting and bill paying, including situations involving
    "spending down" to obtain Medicaid eligibility.

  • TLCCS provides a variety of bill paying and paperwork services.

  • We can prepare checks for client signature, reconcile checking accounts, and prepare and make
    bank deposits,

  • TLCCS will review medical insurance papers and verify the proper processing of claims

  • We will analyze health care needs during Medicare open enrollment and recommend the best medical
    and prescription Part D program available.

  • TLCCS will organize financial documents and negotiate with creditors.

    In addition, TLCCS can become conservator, trustee, or agent under a power of attorney, or guardian in
    guardianship cases when no realistic options are available.

MEDICATION MANAGEMENT:

  • Medication monitoring
  • Coordinate medical and other designated appointments
  • Participate at facility care planning meetings
  • Assistant clients with medication compliance
  • Arrange for equipment (healthcare alert devices, grab bars, bathroom and bedroom
    devices)
  • Pharmacy analysis including review of all prescribed and over-the counter medications

 PHYSICAL:

    ● Provide home visits to support and monitor the effectiveness of the present care plan.
    ● Assist in finding alternative living arrangements
    ● Locate needed medical services
    ● Provide information and referral
    ● Liaison with families at a distance, overseeing care and keeping families informed
    ● Provide consumer education, negotiation, and advocacy
    ● Assess programs that support family caregivers
    ● Create attainable goals and support action towards realizing them

 EMOTIONAL

    ● Improve communication and relationship among family members
    ● Design effective interventions that improve family decision-making
    ● Support completion of unresolved matters


SOCIAL

    ● Encourage disabled adults or older adults to engage in enjoyable activities
    ● Provide support systems
