A Care Manager is professional who evalutes, advises and guides clients and their families through the maze of long term care services and provider options. Care Managers are concerned with the logistical implementation of an individualized care plan and the services that will promote positive outcomes.
A patient advocate is a professional who provides services to patients and those supporting them who are navigating the complex healthcare continuum. Advocates work directly with clients (or with their legal representatives) to ensure they have a voice in their care and information to promote informed decision making. Advocates may work independently or in medical or other organizational settings. They serve individuals, communities, disease‐specific populations, and family caregivers. Total Long-Term Care Consultant Services recognizes the value of employing the principles of patient advocacy and care management coordination to provide the collaboration of care that our clients deserve. We do not identify a specific population for our services, but rather, we choose to focus on the holistic approach that is critical for persons challenged by medical crises.
Our assessments include long-term care planning that addresses Behavioral, Cognitive, Home Environment, Medical status, Safety, Family, Caregiver and Financial concerns.
When the client and family need support on an on-going basis, the Care Manager is an advocate who provides oversight, coordination, and support for the client and family.
TLCCS can manage the bill paying, payroll preparation for your private duty (independent contractor) employee, forms and filing with your insurance company. We can also assist in document gathering for benefits.
TLCCS can work with your pharmacist, physicians, and behavioral medicine practitioner to improve therapeutic outcomes.
We can assess skills and assist the current caregivers when needed. TLCCS will assist in finding alternative living arrangements when changes are needed.
When skilled therapies may benefit our clients following functional decline, we will work with the physician to obtain an order for the therapy as appropriate: physical, speech, occupational, respiratory therapies, etc.
Facilitate communication between family members and interested parties in order to improve decision-making.

Trusteeship: Management of funds and/or property held by one person (the trustee) for the benefit of another (the beneficiary).
Special Needs Trusts: A trust allows the beneficiary to maintain eligibility for public support and receive other federal or state benefits.
Representative/Protective Payee: We accept delegation by the Social Security Administration, Veterans Administration or other government agency to receive the income of a person for bill paying and financial management.
