Work with pharmacist and physicians on medication monitoring for side effects, over the counter medications and ensuring discontinued medications are removed from access.
Coordinate medical provider appointments and ensure medication reconciliation between physicians.

Arrange for Consumable and Durable Medical Equipment (DME), healthcare alert devices, grab bars, bathroom and other assistive devices.
Design effective interventions that improve family decision-making of unresolved matters.
Encourage clients to engage in enjoyable activities.
Coordinate access to Behavioral Management Practitioners to prevent institutionalized crisis care.
Assist in providing spiritual support through community churches, synagogues, mosques, temples, and etc.
Respect personal orientation preferences.
Telehealth support by providing and participating in video appointments with clients.
Home Safety Assessment provides 240-pt. checklist of safety concerns inside and outside the home.
